- Any relevant test results
TAC and WORKCOVER patients
- Employer name, phone number and address
- Claim number (if allocated)
- Date of injury (if applicable)
- Insurance company name
Non-english speaking patients
Please bring a family member or friend who can interpret at your consultation. The use of children as interpreters is not encouraged.
If a professional translator or interpreter is needed, please inform our staff when booking your appointment.
